Frequently Asked Questions

Cancelations & Missed Appointments

Patients are financially responsible for the full session fee for scheduled sessions which are cancelled by the patient less than 48 business hours ahead of the appointment time.  This policy also applies to missed appointments.  Cancellations should be made by telephone, not email.  Please call the office at 480-392-1729; leave a message if necessary.  Cancellations cannot be made via text message.

Prescriptions

 If medications are needed, I will provide prescriptions during the session.  Please understand that it is your responsibility to notify the provider during the session if your prescriptions are running low and you will need a refill.  Medication changes or refill orders will be made during session if needed.  All prescriptions will be sent electronically to your pharmacy. Calls from pharmacies requesting routine prescription refills requires the provider to review your chart and make notes about the medication management, so additional charges may apply if outside of scheduled appointment.  

Appointments

Appointments Please record our scheduled appointments in a calendar and keep track of appointments.  If you make an appointment through our online system, Kareo, you will typically receive a confirmation email and a reminder email or text message.  We do not use telephone reminders.  Payment for sessions is always due at the time of each appointment.  

Fees & Payments

Fees and Payment currently accepting Aetna, Cigna & UHC, private pay, Health share accounts, & Soon BCBS.

Payment is due at the time of service by cash, check, or credit card. Our office will provide you with a invoice/superbill that you may mail/submit to your insurance company for out of network reimbursement.  The statement will include all the relevant information and codes required by insurance companies for reimbursement.  

What is your new patient procedure?

Please request an appointment online or call the office 480-392-1729. If this appointment is not available you will be notified to choose another time. You will receive confirmation & intake paperwork to fill out, (forms & screenngs). Then you will recieve a reminder via text and emails prior to your appointment . If you need to cancel for any appointments, a 48 hour leeway is advised, to not be charged, there is a 24 hour cancelation policy.
